The Case Management Program provides case management services to eligible seniors who need help at home. It also offers non-medical, in-home aide and ancillary support services.

 

Case Management

 

If you, or a senior you know, are having difficulty functioning independently at home, Case Managers may be able to provide assistance. Case managers will:

 

  • Make home visits to evaluate the situation and assess your needs. This assessment is comprehensive and confidential.
  • Develop a care plan to identify the types of services, the providers and the duration and schedule of service delivery that will meet your needs. This written care plan will be reviewed with you before services are initiated.
  • Make use of all available resources from the community for you, including:
    • Medicaid
    • Food Stamps
    • Senior Housing
    • Friendly Visiting
    • Home Health Aides
    • Meals-on-Wheels
  • Monitor services to be sure they meet your needs.
  • Reassess and modify the care plan when necessary.

 

In-Home Aide Services

 

Eligibility for in-home services is determined by the Case Manager during the assessment process. This process also includes a financial assessment to determine if there will be any fees for these services.

 

In-home services include the assistance of a homemaker and/or a personal care aide, depending on your individual need.

 

Ancillary Services

 

The services in this category include one-time expenditures for "gap-filling" services, items or equipment that cannot be obtained from any other source. These expenditures are authorized by the Case Manager, and may also involve a cost-sharing responsibility.

 

General Information

 

Services are provided, without fee, to senior citizens age 60 and over, residing in Nassau County. Counseling services may be available through other Family and Children's Association programs and are provided on a sliding fee scale basis.

 

The Case Management Program is conducted under contract with the Nassau County Department of Senior Citizen Affairs, with funds from the Federal Older Americans Act through the New York State Office of the Aging
